SWORN OFFICERS

Below you'll find the form(s) required for sworn police officers to purchase "Law Enforcement Only" items from Code 4 at reduced prices. You'll also find instructions for properly completing each form.

Forms must be submitted to us on official agency letterhead and bear an ORIGINAL ink signature of the agency's CLEO or his/her designee.  All firearms purchased on letterhead must be shipped to your agency address.

Individual Officer Purchase Letter
Use this form if you’re a sworn police officer and are purchasing a weapon for duty use or if you're making an individual purchase at the request of your agency.  With proper documentation, duty weapons are offered to sworn officers at reduced prices.
